| Service Description | The Healthy Mothers Healthy Babies program is a community outreach service providing support, linkages and education to pregnant women who are unable to access antenatal care services or require additional support during their pregnancy and up to 6 weeks post delivery. Women who are pregnant and live in the City of Melton or Moorabool Shire and are considered socially disadvantaged, geographically isolated, from culturally or linguistically diverse backgrounds, of Aboriginal or Torres Strait Island descent or less than 18 years of age. |
