Service Description | Volunteer drivers provide door to door transport for eligible consumers to attend medical appointments, shopping or social outings such as the Senior Citizens club or Uniting Church luncheon. Priority is given to transport requests from clients to attend medical appointments. |
Eligibility | Frail, older people and/or younger people with a disability. |
Access hours | Monday to Friday, 9:30am – 6.00pm |
Referral process | Referrals are made to the Volunteer Coordinator for Consumer Transport and may be received from a variety of sources with the consent of the client. These may include: · Self-referral · Family members · HACC or CHSP Services · Local Medical Officers · Aged Care Assessment Team |
Cost to client | The cost of a return trip 19.23 kilometres or less is $15.00. The cost for a return trip, over 19.23 kilometres is calculated at 75 cents per kilometre. This is reimbursed to the volunteer for out of pocket expenses; such as petrol and maintenance of their vehicle. |
