Our Community Health Nurses will support you in your day to day needs. They are available for both home visits and centre based appointments.

Wound Management
Continence Support

General nursing

Dementia Support
Palliative Care (Moorabool Only)

How do I get an appointment

If you are over the age of 65 (50 for Aboriginal and Torres Strait Islanders) please contact My Aged Care on 1800 200 422.

You may have a GP referral to access this service or contact 03 5367 9839.
