Description of Service:

A Diabetes Nurse Educator helps to support a patient with diabetes to manage their sugar levels. They will support you in:

Learning to use devices
Understanding the importance of testing
Understand the condition
Develop management strategies to decrease the risks associated with diabetes

If you are over the age of 65 (50 for Aboriginal and Torres Strait Islanders) please contact My Aged Care on 1800 200 422.
