| Service Description | This service is aimed at clients with early stage chronic disease or risk factors for the development of chronic disease. The Health Coach will work in partnership with the client, family/carer, doctor and other health professionals to support the management of the client's health condition and improve general health and wellbeing. The program operates from a self-management model, whereby the client will be encouraged to make decisions about their health and lifestyle as well as set goals for improvement. The health coach provides support and assistance to make behaviour changes to achieve these goals as well as considering referrals to other services and provide general information about managing health conditions and risk factors. |

| Referral process | Referrals are accepted from General Practitioners, Medical Specialists or other health professionals and can be faxed to our Referral and Appointment Service on 9746 0668 or clients can self refer. |
| Cost to client | Pension or Health Care Card holders pay a reduced fee. Clients who do not hold a pension or Health Care card must complete a income self declaration form to determine fee payable and are charged in line with the Department of Health Primary Health Programs Fee Policy. |
