Service Description | Continence Nurse Consultants works within the Community Nursing team and visits clients in their homes. A full assessment of the client is undertaken, advice is provided on the management of the condition, education and information is also provided, and assistance can be obtained when making applications to the Continence Aids Assistance Scheme and other funding bodies if clients are eligible for products and equipment. The advisor may also refer to other health professionals. |

Access hours | Monday to Friday, 8.00 a.m. – 4.30 p.m. |
Referral process | General Practitioner, Hospital, Aged Care Assessment Service, Regional Assessment Service, National disability Insurance scheme, Transport Accident Commission, family or self referral. |
Cost to client | By client’s General Practitioner, Treating Hospital, Regional Assessment Service, Aged Care Assessment service or self referral. |
Cost to client | Pensioner under 65 years $3.90 per visit. (Capped at $39 per month depending on number of visits and individual circumstances) Over 65 years $5.00 per visit (Capped at $50.00 per month depending on number of visits and individual circumstances) Other fees dependent on self-assessment guidelines provided by the Department of Human Services. |
