| Service Description | The Dementia Support Nurse works with the District Nurse team and visits clients in their own homes. She offers holistic assessments, information, support and education to both clients (with dementia) and their carers. Positive approaches to living with dementia will assist the clients to live as long as possible in their own homes. The nurse may also refer to other health professionals. |

| Access hours | 4 Days a week, 8.00a.m. – 4.30 p.m. |
| Referral process | General Practitioner, Hospital, Aged Care Assessment Service, Regional Assessment Service, family or self referral. |

| Cost to client | Pensioner under 65 years $3.90 per visit. (Capped at $39 per month depending on number of visits and individual circumstances) Over 65 years $5.00 per visit (Capped at $50.00 per month depending on number of visits and individual circumstances) Other fees dependent on self-assessment guidelines provided by the Department of Human Services. |
