| Service Description | • Wound care (simple and complex) • Diabetic care • Catheterisation • Administration of medications (including injections) • Stomal care • Drain tube management • Complex medical care post discharge from hospital and some more complex personal care to clients in their own homes. Area covered includes Bacchus Marsh and Melton and surrounding areas. |
| Eligibility | Any person requiring a district nurse. |
| Access hours | 8.00am – 6.30pm, 7 days a week. |
| Referral process | By client’s General Practitioner, Treating Hospital, Regional Assessment Service, Aged Care Assessment service or self referral. |
| Cost to client | Pensioner under 65 years $3.90 per visit. (Capped at $39 per month depending on number of visits and individual circumstances) Over 65 years $5.00 per visit (Capped at $50.00 per month depending on number of visits and individual circumstances) Other fees dependent on self-assessment guidelines provided by the Department of Human Services. |
