Service Description
Following delivery of your baby, and discharge home, one of our Midwifery team will visit you at home to ensure the transition into home life with your new baby is as smooth as possible.
This service monitors the wellbeing of both mother and baby until you are contacted by the Maternal and Child Health Nurse service (usually within a week of discharge).
At least one home visit is provided to all new Mums. If you have gone home less than 24 hours after delivery, or require additional support, extra visits may be arranged.

Eligibility
All Mothers who have delivered at Bacchus Marsh Hospital.
Mothers who reside in Moorabool or Melton and have delivered at other Health Services (by arrangement from the hospital of delivery).
