What is this service?

 Children residing in the statutory Out of Home Care (OoHC) system have experienced a level of child abuse and neglect that has meant that they are no longer able to live safely with their families. As well as experiencing significant trauma in their lives, these children also have health outcomes that are much poorer than the general child population.

To ensure that these highly vulnerable children receive the best possible health care to address health and wellbeing needs, a model of coordinated care and health planning has been developed and funded by the Department of Families Fairness & Housing (DFFH) known as the Pathway To Good Health. For more information click here.Our Pathway to Good Health clinic at Melton contains a specialised multidisciplinary team that includes a Paediatrician, PaediatricSpeech Pathologist and Paediatric Psychologist.The role of the Pathway to Good Health team is to provide a comprehensive assessment of the child and develop a health management plan that will include any diagnosis and recommended treatment or referrals.Children are referred to the Pathway to Good Health clinic by the child protection or Out of Home Care agency working with the child and a General Practitioner.




Who can use this service?

Children and young people (0-16 years old) in the Western Suburbs of Melbourne living in OoHC (kinship, foster or residential care)who are being actively supported by child protection and who are not already connected with paediatric care.




How much does it cost?

This is a free service.




How do I make an appointment?

To obtain a referral to the Pathway to Good Health clinic, please attend a health screening appointment with a General Practitioner (GP). You can find more information for General Practitioners here
