When you have your NDIS plan, contact us and we will register a referral. This referral connects you with us.

Once a referral is registered someone will be in touch to develop a service agreement with you
To help us work with your towards your goals, you will be asked if you are happy to share your NDIS plan and personal information with us.
We may contact you by mail phone and by email.
If ever you think you are speaking to someone who is not from Western Health Service (when they have told you they are), do not tell them your personal information.

We develop a service agreement with you and your supports so you know what we provide. We work in partnership with you and your wider support network.
It’s never a one-size-fits-all solution. Together, we’ll decide on the best way to support you, so that you can continue building a happy and healthy lifestyle.
A copy of your service agreement will be given to you and placed on your Western electronic medical record.
When the Service Agreement is complete, an appointment is made with the services you have chosen.
You can ask someone you know to be an support or nominee. Like your family or your friend or someone from your community, If you do not have a nominee and need someone to support you to have your wishes or your concerns heard you can access help using an advocate.

Each service will work with you to complete an assessment so you can decide the things you want to work on and the supports you want.
Our services operate with a mixed model approach. This means, where safe we will support you with telephone and telehealth (Video). We also offer face to face services. You will be told how the service will be delivered.
We will provide reports on your progress as part of our requirements with NDIS. This includes an one as part of your beginning with our service and an end of plan report.
